The 11033L hub shown in your photo was manufactured by Tie Down Engineering, or TDE. What you have there is a hub for a standard 3500 lb axle which has a 5 on 4-1/2 inch bolt pattern. For an idler hub, I'd recommend the galvanized # AKIHUB-545-35-G-K. The hub includes the bearings, races, grease seal, dust cover and lug nuts.

I have added grease through the zerk fitting but it does not help.
